Person Centred Care

As the phrase “person centred” states, a person-centred approach is about making sure that a vulnerable person is at the centre of  all decisions which relate to their life. A person-centred process involves listening,  collaborating, thinking together, coaching, sharing ideas, and seeking feedback from everyone important to the individual.

Person-centred care is a way of thinking and doing things that sees the people using health and social services as equal partners to co-create and plan, develop and monitor the care and support to ensure all needs are met. This means putting people and their families at the centre of decisions and seeing them as experts, working alongside professionals to get the best outcome.

 

Person-centred care is not just about giving people whatever they want or providing information. It is about considering people’s desires, values, family situations, social circumstances and lifestyles; seeing the person as an individual and working together to develop appropriate solutions. Being compassionate, thinking about things from the person’s point of view and being respectful are all important. This might be shown through sharing decisions with people supported and helping people manage their care package, but person-centred care is not just about activities. It is as much about the way professionals and patients think about care, support and their relationships as the actual services available.

In order to be person-centred, services must adapt to be more flexible to meet people’s needs in a manner that is best for them. This involves working with people and their families to find the best way to provide their care.

This partnership works best on a one-to-one basis, where individual people take part in decisions about their care & support, or sometimes on a collective group basis whereby ‘appropriate’ others are involved in decisions about the design and delivery of services.

The Team

At Unity Lives we understand the importance of skills matching our staff to an individuals needs, we will always endeavor to build a team around each individual that ensures that the individual is happy and comfortable in all activities.

In some instances individuals will be part of the recruitment process, truly putting them at the forefront of deciding who delivers their support and care. 

All of our teams will receive mandatory training and bespoke training packages to enable them to deliver the best quality of care to the individuals they are working with
